Evans St. Sheboygan, WI 53081 wallyhanna@rcn.com 920-452-1761 Request For Information / Quotation An initial market test quantity of die cut plastic discs with a welded-on pull tab. Polypropylene or other least expensive material that latex and oil-based paints will not adhere to. Approximately 1/32” thickness. Very flexible, but not floppy. Transparent or very translucent. Each disc will have a pull tab, die cut from the same poly sheet to minimize waste. The tab must be press-creased to be easily bent up from flat by the user, and then hold an “up” position. The tab must be securely “spot welded” to the disc. The product name must be printed on the disc top side in generic red: screened, hot pressed, or otherwise. Initial Quantity: 1000 of each disc, 6.5” diameter +/- and 4.125” diameter +/-. Final dimensions will be set after sample and before production. Design cutting dies to minimize poly sheet waste. Assume recycle of waste poly in pricing. Provide initial evaluation samples of both diameters in readily available standard poly sheet thicknesses: Two thicknesses over and two thicknesses under the base 1/32”. Printing not required. One of each equals 10 samples required. OPTIONAL: 1. Provide 333 clear poly packages containing 3 discs of each size: A 7” W x 9” H clear flat poly bag, finish sealed with a ?” top retail hook hole, printed two sides in black. Package graphics provided in digital form. 2. Provide 4000 printed 2.75” dia. peel-off back adhesive labels. Insert 12 into each of 333 poly retail packages. Graphics provided in digital form.
